ACA-compliant coverage
All 10 Essential Health Benefits required by the Affordable Care Act — including maternity, mental health and prescriptions.
Health Benefits
ACA-compliant PPO coverage for your clients’ teams and individuals — national network access, without the narrow rules of an HMO or the public exchange.
Cennairus is a licensed wholesale insurance broker: retail agents send us the accounts their own markets cannot place, stay the agent of record, and we take each account to the markets on our panel of 145 carriers that write it. Health benefits through Cennairus are ACA-compliant PPO plans for an employer’s team and for individuals, with national network access, out-of-network care and no referral needed to see a specialist. There is no minimum participation mandate, so a policy is available for one or more lives, and the four plan options include medically underwritten and non-underwritten plans.

Designed around businesses whose headcount moves, without a strict minimum-participation rule to trip over.
In plain terms
Health plans cover core medical costs—including doctor visits, hospital stays, and prescriptions—for both employer groups and individual families. ACA-compliant major medical policies incorporate the ten federally required Essential Health Benefits, keeping critical services like maternity care, mental health, and prescription drugs unified under a single plan rather than carved out into separate coverage.
How the plan responds depends heavily on network design. A PPO plan pays for care outside its network, at a higher cost-share, and generally does not require a referral from a primary care doctor before seeing a specialist — both of which a narrower HMO or EPO plan usually restricts.
Health benefits are a baseline requirement for competing in today’s job market, standing as most small businesses’ largest recurring expense behind payroll. For those outside an employer offer—such as individuals in job transitions or aging off family plans—direct coverage fills the gap. Though the underlying plan options often mirror group benefits, individual setups follow a separate enrollment path.

Plans at a glance
Plan alternatives to the outdated rules of the traditional health insurance carriers. Our ACA-compliant, enterprise-level PPO plans carry no mandatory carrier participation requirement.
Underwritten Medically underwritten — approval is based on the individual or group’s health history
Aetna Open Access POS II
The broadest provider access — more than 1.4 million providers. Available with or without deductibles.
Anthem Blue Card + Northwell Direct
A full national PPO with free virtual care included. Available with or without deductibles.
Not underwritten Guaranteed approval — no health history is asked to qualify.
PHCS MultiPlan
Minimum-value coverage. From $549 a month, with $0 deductible plans available.
Anthem National Blue PPO
Five options. From $1,189 a month, with deductibles from $0 to $3,000.
Every plan needs no referrals and carries a large or unlimited maximum benefit, with no minimum participation. The trade-off is premium against deductible — richer copay plans cost more, while base plans use a deductible and coinsurance for a lower premium. The non-underwritten pricing above is national: the same in all fifty states.
Benefits for business owners?
Joining a larger group lets a business reach Fortune 500-level benefits and large-group pricing — advantages usually out of reach for small and mid-sized companies on their own.
A digital personal health questionnaire and a HIPAA form from each applicant give us permission to work the quote.
A group census with each person’s name, gender, ZIP code, date of birth and dependent coverage preference.
Once the forms are complete, underwriting comes back in one to three business days, depending on the group size and the state the business is in.
Why choose us
Traditional individual plans have become increasingly narrow, often excluding top-tier hospitals and specialists. Our PPOs prioritise access to gold-standard facilities nationwide.
You do not need a referral from a primary doctor to see a specialist. That saves time and removes the administrative hurdles common in traditional HMO plans.
Because these plans operate via larger pools, they often offer more stable year-over-year rate increases than the volatile individual marketplace.
Group members get dedicated customer support and advocacy to help navigate claims through the plan’s TPA — a common pain point in traditional plans.

Worth knowing
A medical plan answers for health costs specifically — each of these is a different policy, and we place all of them.
Treatment for a workplace injury is a Workers’ Comp claim, even for an employee enrolled in the group plan.
Employment-related claims sit under EPLI, not the health plan.
That is General Liability — a plan member’s health coverage does not extend to third parties.
A domestic plan is not built for care outside the country. Travel picks up emergency medical and evacuation abroad.
The same conversation that renews the property and the comp is where the health option belongs.
Questions we get
Yes. Our Horizon plans and Harbor plans are underwritten, and our non-underwritten plans are Cove plans & Bay plans.
Out-of-network access and no referral requirement to see a specialist. An HMO or EPO plan generally restricts both.
This program is built around no minimum participation requirement, so a high-turnover team is not disqualified the way a standard small-group filing can be.
A census — name, ZIP, date of birth, and employee or dependent — for a group, or basic household information for an individual application. The census template is above.
